If you fail to file a Petition to Legitimate within thirty (30) days of your receipt of this notice; file a legitimation petition which is subsequently dismissed for failure to prosecute; or file a legitimation petition and the action is subsequently concluded without a court order declaring a finding that you are the legal father of the child, then this Court shall enter an order terminating all your parental rights to the child and you may not thereafter object to the termination of your rights to the child.

This Court will conduct a provisional hearing upon the allegations of the Motion for Extension of Temporary Custody and enter an interlocutory order of disposition pursuant to O.C.G.A. § 15-11-39.2 on the 4th day of February, 2013, at 1:30 p.m., at the White County Courthouse, Cleveland, Georgia. Parties to this action are required to appear and answer the allegations of this Petition at the provisional hearing. The fmdings of fact and orders of disposition made pursuant to the provisional hearing will become final at the final hearing unless you appear at the final hearing.

This Grand Jury received a report from Rita Harkins, Clerk of Court, concerning receipts and disbursements of the fiscal operations of the Superior Court Clerk’s Office. Lumpkin County Sheriff’s Office

This Grand Jury received a report from Stacy Jarrard, Sheriff concerning fiscal operations of the Lumpkin County Sheriff’s Office. The report presented by Sheriff Jarrard is that required by O.C.G.A. § 36-1-7(a) and was found to be correct. All funds involving the Sheriff’s Office are handled through the County Commissioner’s Office and are accounted for in the Commissioner’s report.

Lumpkin County Probate Court

This Grand Jury received a written report from the office of Michael Chastain, Probate Court Judge. The report by Judge Chastain’s office is that required by O.C.G.A. § 36-7-l (a) and was found to be correct. A copy of said report is attached as part of these minutes.

Lumpkin County Tax Commissioner

This Grand Jury received a report from Rachel Pruitt, Tax Commissioner, concerning the Tax Execution Docket and cash book. The return presented by Ms. Pruitt is that required by O.C.G.A. § 48-5-l6l(d) and was found to be correct.

Lumpkin County Commissioner’s Office

This Grand Jury received a report from Allison Martin, County Finance Director, and

Stan Kelley, County Manager, concerning the receipts and disbursement of the fiscal operations of the Office of County Commission. The return presented by Allison Martin and Stan Kelley is that required by O.C.G.A. § 36-1-7(a) and was found to be correct. A copy of said report is attached as part of these minutes.

The Grand Jury inspected the Lumpkin County Detention Center and was given a tour of the facility and everything was found to be in order.

The Grand Jury reviewed the Lumpkin County Detention Center’s Jail Book and found it to be kept correctly, pursuant to O.C.G.A. § 42-4-8.

Lumpkin County is applying for funding assistance under Title 49 U.S.C. Section 5311 of the Federal Transit Act pertaining to rural and small urban areas.

Lumpkin County will offer general public transportation to all citizens of Lumpkin County for any worthwhile purpose including but not limited to shopping, medical treatment, social services and other purposes.

Lumpkin County solicits private sector input and participation to assure that private for-profit transportation operators have a fair and timely opportunity to participate in the development of this program.

Lumpkin County also solicits comments and concems from the general population on local rural public transportation services.

Lumpkin County also solicits comments and concems from the disabled population and their representatives to assure that issues relating to the disabled are addressed in the service design proposed during the

planning process.

Interested persons are invited to request that a public hearing be held to discuss the services being offered or development of the application.

If you fail to file a Petition to Legitimate within thirty (30) days of your receipt of this notice; file a legitimation petition which is subsequently dismissed for failure to prosecute; or file a legitimation petition and the action is subsequently concluded without a court order declaring a finding that you are the legal father of the child, then this Court shall enter an order terminating all your parental rights to the child and you may not thereafter object to the termination of your rights to the child.

This Court will conduct a provisional hearing upon the allegations of the Motion for Extension of Temporary Custody and enter an interlocutory order of disposition pursuant to O.C.G.A. § 15-11-39.2 on the 4th day of February, 2013, at 1:30 p.m., at the White County Courthouse, Cleveland, Georgia. Lumpkin County will offer general public transportation to all citizens of Lumpkin County for any worthwhile purpose including but not limited to shopping, medical treatment, social services and other purposes.

Lumpkin County solicits private sector input and participation to assure that private for-profit transportation operators have a fair and timely opportunity to participate in the development of this program.

Lumpkin County also solicits comments and concems from the general population on local rural public transportation services.

Lumpkin County also solicits comments and concems from the disabled population and their representatives to assure that issues relating to the disabled are addressed in the service design proposed during the

planning process.

(viii) All right, title and interest of Grantor in and to all options and contracts to purchase or lease the Premises or any portion thereof or interest therein, and any greater estate in the Premises owned by Grantor;

(ix) All accounts receivable, contracts and contract rights (including, but not limited to, Grantor’s interest in any and all leases, tenant contracts, rental agreements, franchise agreements, management contracts, construction contracts, architectural contracts, purchase contracts, consulting contracts, engineering contracts, brokerage agreements and other contracts, licenses and permits affecting the Premises), chattel paper, documents, interests, estate or other claims, both in law and in equity, which Grantor has in the Premises;

(x) All proceeds of and any unearned premiums on any insurance policies covering the Premises, including, without limitation, the right to receive and apply the proceeds of any insurance, judgments, or settlements made in lieu thereof, for damage to the Premises;

(xi) All the estate, interest, right, title and other claim or demand which Grantor has against anyone with respect to any damage to all or any part of the Premises, including without limitation, damage arising from any defect in or with respect to the design or construction of all or any part of the improvements and any damage resulting therefrom;

The indebtedness secured by the Security Deed has been and is hereby declared due because of default under the terms of the Note and the Security Deed, including but not limited to, the nonpayment of principal and interest when due.
